We strive to see patients on time.  Please arrive at least 15 minutes prior to your scheduled appointment time and bring your insurance card and completed paperwork.* We now have the forms necessary for your visit available online. You may click on the links available on this page (FORMS), which can be directly accessed through the PATIENT PORTAL. These forms (see below) are also available through this PORTAL under the heading: YOUR FIRST VISIT or any other necessary FORMS the Doctor has directed you complete, and can be printed out and completed prior to your visit. Completion of the following forms in advance will help us to achieve our goal of being more efficient with your time in our office.
